Take the Test for Chlamydia and Gonorrhea (This test does not test for all STIs.)

If I test positive for chlamydia or gonorrhea, pursuant to the Health Protection and Promotion Act, I am aware that I will be contacted by a Public Health Nurse, require treatment (provided for free at the Sexual Health Centre) and asked to provide information about my sexual partners.  This information will be used to contact these partners to ensure that they are adequately tested and treated; my name will not be released to these partners. Confidentiality of information obtained is assured.
